Test Center Application Form

This form is to be completed by new testing centers as part of the onboarding process.

There are 4 pages within this form, including a document upload section on the last page.

If necessary, you can save the form at any time and complete it later.

We recommend that you have all of the following prior to starting this form:

Measurements of desks in the exam room
Center seating plan to upload (ensure this shows the candidate facing direction and that every desk/computer is numbered)
Internet speed test of testing computers
Candidate journey video (this can be filmed on your phone and should show the candidate entering your center):
- Start from the entrance door to the building, panning the outdoor area
- Move through the center to the exam room
- Ensure you are recording bathrooms, lockers, reception etc on video so we can see the full candidate journey that they would experience on exam day
- You don’t necessarily need to narrate the video, and you don’t need to edit it. We will do any editing needed on our end once received

Please note: If you are unable to take a candidate journey video, we will accept detailed photos of your center (externally and internally, including lockers, bathroom locations and multiple of the exam room). A candidate journey video is preferred.
